Our care professionals will provide you with the support that you need.

Our Personal Support at Home4Me, Inc. would offer personal assistance, supervision/support, and training for individuals living in their own homes or the family home. Personal Supports would offer a range of home and community-based services intended to assist the person to participate fully in home and community life.

Personal support services are also known as Community Supported Living Arrangements (COMAR 10.22.01) and are provided based upon the needs as articulated in the plan of care. These services are provided to individuals with a developmental disability (COMAR 10.22.12) who are currently enrolled in the waiver, are in crisis situations; are moving from a state residential center (SRC), nursing facility, or a mental health facility placement; or are on the waiting list for DDA services and identified as needing services.

These services are provided regularly based upon a specified number of hours required weekly. Personal supports include, but are not limited to:

Hands-on assistance, prompting, and cueing that enable the participant to use assistive technology or accomplish tasks they are unable to complete alone due to a physical disability, including assistance with activities of daily living, including:

  • Completing personal hygiene routines such as bathing.
  • Toileting, including bladder and bowel requirements, bedpan routines, and routines associated with the achievement or incontinence care and maintenance, and movement to and from the bathroom.
  • Mobility, including transferring from a chair, bed, or other structures, and moving around both inside and outside of the house.
  • Moving, turning, and positioning the body while in bed or a wheelchair.
  • Eating and preparing meals.
  • Dressing and changing clothes.
  • Light housework, including laundry for a participant unable to complete a task
  • Preventive maintenance and cleaning of adaptive devices.

Support, supervision, and training may also be provided in such activities as:

  • Housekeeping.
  • Planning the menu, shopping for food, preparing meals, and feeding assistance.
  • Personal care that includes maintaining hygiene and grooming.

Supports to implement behavior plan strategies and at-home therapies as prescribed by a professional.
